Ages and Stages Questionnaire (ASQ)
The Ages & Stages Questionnaire (ASQ) is a developmental screener which assesses five areas of development: communication, gross motor, fine motor, problem solving, and personal-social. This tool will be completed by the parent/guardian of each HS and EHS child within 45 days of enrollment.

Process for completing ASQ Screening:
- Introduce each family to the ASQ. Try using the How to Introduce the Questionnaires to Parents document in the ASQ Resources section below.
- The director of your Collaborative Center will receive information about the Brookes Ages and Stages online account in their email. The director or teacher will send a link for the ASQ-3 screening to the parent/guardian that can be found under the Family Access tab in their account.
- For HS, the director or teacher will also send a link for the ASQ:SE-2. (EHS families complete the DECA instead of the ASQ:SE-2)
- The director will ‘accept’ the screening completed by the family within their Brookes account. Refer to the Accepting Screens Submitted Through Family Access document below for further guidance.
- Your CCSC will download a report from the online account, so you will not need to submit anything.
- The CCSC will also share screening summaries with teachers/directors at the monthly meetings and discuss next steps for children falling within the Monitor or Refer sections.
