Next Steps Based on Screening Results
Early Head Start
ASQ Re-screen
When a child has one or more developmental area falling into the gray scoring section, provide activities to support growth in those areas and complete an ASQ re-screen within 6-8 weeks. If the child scores within the gray section again, consult with the child's parents as well as your Coordinator to discuss next steps for the child. |
ASQ Referral
If a child has one or more developmental area falling into the black scoring section, or if you or the parent suspect developmental delays or disabilities, refer to Early On. With parents, complete the online referral form (https://1800earlyon.org/online_referral.php). The purpose of this program is for families and ISD staff to work together in identifying, coordinating and providing needed services and resources that will help support growth and development of the child. DECA Re-screen and Referral
- No areas of concern re-screen annually
- One area of concern re-screen in 3-6 months
- 2-3 areas of concern re-screen in 6-8 weeks
- Contact the Mental Health Manager if concerns remain and/or are serious in nature
Head Start ESI-R, Refusal, Re-screen, and Referrals
ESI-R Refusal and Re-screen
When a child refuses (CNT) to complete the screening or scores in the re-screen range, complete a re-screen within 8-10 weeks of the original screening date and inform the parent that the child will be screened again.

ESI-R Refer
When a child scores in the refer range, consult with the parent and the Coordinator to discuss next steps for the child. If it is determined that a referral is appropriate, work together with the parent to complete the Request for ISD Consultation form found below. At this time, have the parent sign the Disclosure with Parental Consent form and turn into your Coordinator. This will allow them to request a copy of the child's IEP if the child is found eligible for services. Interim Services Plan
While waiting for results from a referral, complete an Interim Services Plan with the parent and Coordinator to create a plan to support the child. This form can also be used to support children when there is a concern that does not meet the threshold to receive services from the ISD.
