Next Steps Based on Screening Results
Early Head Start
If you or the parent suspect developmental delays or disabilities, refer to Early On. With parents, complete the online referral form (https://1800earlyon.org/online_referral.php) or Request for ISD Services form under the Head Start Refer section on this page. The purpose of this program is for families and ISD staff to work together in identifying, coordinating and providing needed services and resources that will help support growth and development of the child. Head Start ESI-R, Refusal, Re-screen, and Referrals
ESI-R Refusal and Re-screen
When a child refuses (CNT) to complete the screening or scores in the re-screen range, complete a re-screen within 8-10 weeks of the original screening date and inform the parent that the child will be screened again.
  1. If the child still refuses the screening, the teacher, parent, and Coordinator, together, decide next steps for the child. At any time during this process, the parent may request a referral. 
  2. If a child scores in the re-screen or refer section for the second time, consult with the parent and Coordinator to discuss next steps for the child. If the parent agrees to make a referral, the teacher and parent will work together to complete the Request for ISD Consultation form found under the Refer section on this page.
ESI-R Refer
When a child scores in the refer range, consult with the parent and the Coordinator to discuss next steps for the child. If it is determined that a referral is appropriate, work together with the parent to complete the Request for ISD Consultation form found below. At this time, have the parent sign the Disclosure with Parental Consent form and turn into your Coordinator. This will allow them to request a copy of the child's IEP if the child is found eligible for services.

Interim Services Plan
While waiting for results from a referral, complete an Interim Services Plan with the parent and Coordinator to create a plan to support the child. This form can also be used to support children when there is a concern that does not meet the threshold to receive services from the ISD.
