EHS ASQ Referral
If a child has one or more developmental area falling into the black scoring section, or if you or the parent suspect developmental delays or disabilities, refer to Early On. With parents, complete the online referral form (https://1800earlyon.org/online_referral.php). The purpose of this program is for families and ISD staff to work together in identifying, coordinating and providing needed services and resources that will help support growth and development of the child. When a child scores in the refer range, consult with the parent and the Coordinator to discuss next steps for the child. If it is determined that a referral is appropriate, work together with the parent to complete the Request for ISD Consultation form found below. At this time, have the parent sign the Disclosure with Parental Consent form and turn into your Coordinator. This will allow them to request a copy of the child's IEP if the child is found eligible for services. Interim Services Plan
While waiting for results from a referral, complete an Interim Services Plan with the parent and Coordinator to create a plan to support the child. This form can also be used to support children when there is a concern that does not meet the threshold to receive services from the ISD.

What if the family does not complete the screening after I have sent the link a couple of times?
- Check in with the family to see if there are barriers preventing them from being able to complete the screening. Do they have technology, internet connectivity, or illiteracy challenges? Maybe they do not have the materials needed to complete the screening at home. You might try:
- Offer to let the family complete the screening at the center before, during, or after classroom hours.
- Provide the classroom iPad for the family to complete the screening at the center.
- Provide a paper copy of the ASQ for the family to complete the screening at home.
- If they are feeling uncertain about their ability to complete it, ask the family if they would like you to help them complete the screening.
- If the parent/guardian is unable to complete the screening after your attempts to remove barriers and support the family, you can ask the family if they would like you to complete the screening.

- No areas of concern re-screen annually
- One area of concern re-screen in 3-6 months. Refer to the Strategies report for ideas to support the child's social/emotional development at home and in the classroom.
- 2-3 areas of concern re-screen in 6-8 weeks. Refer to the Strategies report for ideas to support the child's social/emotional development at home and in the classroom.
- Contact the Mental Health Manager if concerns remain and/or are serious in nature
