Early Screening Inventory (ESI-R)
The Early Screening Inventory-Revised (ESI-R) is a brief developmental screening instrument that is individually administered to children aged 3 through 5 years. It samples performance in the areas of speech, language, cognition, perception, and motor coordination. Children are screened within the first 45 days of enrollment.
ESI-R Composite form
Children enrolled before their third birthday, and who will not turn three within 45 days of enrollment, will be screened using the ASQ. Talk to your Coordinator if you have any questions about the ASQ.
- Children enrolled in their second year of HS do not need to be re-screened.
- Use the ESI-P form for children ages three through four years, six months.
- Use the ESI-K form for children ages four years, six months to six years.
- Parents complete the ESI-R Parent Questionnaire form to satisfy the social-emotional screening component.
- Children with an existing IEP will receive the screener but do not need to be re-screened or referred.
